| At Crestwood Care Centre a holistic plan of care is designed for our wound patients. Our staff is trained to anticipate and care for their needs.
Upon admission, the needs and concerns are assessed and individualized for each patient. The plan of care is designed and appropriate treatment modalities are integrated in the care such as ultra sonic mist, ultraviolet light, pulse lavage, negative pressure and/or electrical stimulation therapy. A nurse coordinator, assigned to each patient, works closely with family and physician to monitor progress and serve as a resource to address concerns. They collaborate with the discharge physician’s hospital to ensure appropriate follow up care is coordinated on discharge from the facility. Our interdisciplinary team, physician specialist, wound ostomy nurse, dietician, social worker, physical therapist, occupational therapist, speech therapist, and nursing staff, assess and reassess the plan of care. This has reduced our average recovery time and stay by 2 – 8 weeks.
