Crestwood Care Centre’s Stroke Recovery Program is designed to build physical strength and mental well being. A post-stroke patient requires a coordinated team approach to rehabilitate processes on a timely basis to restore independence. A customized program is implemented by our team of nurses, therapists, nutritionist and social workers to work in tandem with our neurologist and physiatrist.

Treatments include:

                   • Physical, occupational and speech therapy

                   • Gait, balance and mobility training

                   • Psychological and social support

                   • Bowel and bladder management

                   • Dysphagia therapy

                   • Social interaction

In collaboration with the attending physician, our nurses provide continuous nursing care including vital sign monitoring, administration of medicines and treatments and assistance with activities of daily living. Our staff provides a supportive environment and educational materials that allow patients and their families to learn how to adjust to the disability a stroke may cause.

We offer a seamless transition from the acute hospital setting to our skilled units by a clinical nurse liaison who communicates vital information between both settings.

Our objective is to prepare the patient for a functional return to the home and community.
